my project is on fall prevention among older adults age 60 an above residing in long-term care facility. Goal met – educating above 90 of the staff on fall prevention protocol, risk assessment, and overall project goals; successfully implementing the John hopkins fall risk assessment on patients meeting criteria for inclusion. Barriers – how to successfully provide teaching to the staff since they work different shifts. Solution was to provide teachings at different times to include morning, evenings, night and weekend to ensure that 80% of staff were aware of the protocol. Current status project status -adoption of John hopkins fall risk assessment for patients meeting inclusion criteria, staff providing teachings to patients/families on prevention, having fall risk signals to communicate patient status, and huddles held post fall to understand root cause, and teachings still ongoing
